INSTRUCTIONS

1) Read and understand the request letter. If you don’t understand, or it does not fully apply, don’t sign.
2) Complete and modify as needed to suit your circumstances.
3) Sign.
4) Make copies, reserving one for the U.S. Special Prosecutor’s Office in case.
5) Prepare to send by certified mail. Take one photo of the letter, addressed envelope, and certified mail form all together just prior to mailing.
6) Send via certified mail.
7) Get an appropriate response, or wait for deadline to pass.

Re: 18 U.S. Code § 3332 Request for Grand Jury Process

Greeting.

Please be informed that I am aware of, and I allege that one or more serious offenses against the criminal laws of the United States have been committed in the district, resulting in the necessity of a federal grand jury inquiry. (If applicable: “I am a crime victim, having been injured as a direct and proximate result of one or more of the alleged offenses, and/or I legally represent such person who isn’t one of the accused. I am entitled to all the rights of a crime victim applicable to this request.”) To the best of my knowledge and belief, none of the matter alleged has already been tried or committed to any criminal court proceeding; I don’t believe any statute of limitation has expired on lawful and timely enforcement; and no means of remedy on the state level is more proper or reasonably available. I have evidence and/or testimony supporting my allegation(s), and under 18 U.S. Code § 3332, it is the federal grand jury’s duty to inquire into the allegations. I respectfully request that you inform an appropriate grand jury in the district of the alleged offenses, the testimony and/or evidence due for their inquiry, and my contact information as provided below. You may provide them with this letter. I trust the grand jury will have already been informed of their duty to inquire, and you’ll take all appropriate actions and make all appropriate recommendations in consideration of our dependence on you in this process, as without this, due process may fail. I also trust that you’ll promptly inform me of the satisfaction of this request, or any exception to it, considering that without this, I’ll be left to assume, at least by (date), some disability or obstruction, relegating me to seek some other remedy, as all crime victims are entitled to remedy wIthout delay.
